Terms and Conditions of Use

Effective Date: April 14, 2026

PLEASE READ THESE TERMS OF USE CAREFULLY BEFORE USING THIS SITE.

These Terms and Conditions of Use (“Terms”) reflect the agreement that operates between you (“Site User”), and Prestige Veteran Medical Consulting, LLC (“PVMC”), a Texas limited liability company, and govern the use of the platforms provided by PVMC, including any websites on which these Terms are displayed (such platforms referred to herein as the “Site”). These Terms describe the rights and obligations of all Site Users regarding the use of the Site. PVMC may immediately terminate these Terms or any Site with respect to Site Users, or generally cease offering or deny access to the Site or any portion thereof, at any time for any reason, except where prohibited. Site User’s access to and use of the Site is conditioned on Site User’s acceptance of and compliance with these Terms. These Terms apply to all visitors, users, and others who access or use the Site (collectively, “Site Users”).

  1. Who May Use the Site

Site User may use the Site if of legal age to form a binding contract in Site User’s jurisdiction and is not barred from doing so under applicable laws. In the United States, Site User must be at least eighteen (18) years old to use the Site and to engage PVMC’s services. By using the Site, Site User agrees to comply with these Terms and all applicable laws and regulations. It is Site User’s responsibility to provide accurate and current information and to maintain the security of Site User’s account. PVMC’s services involve medical record review and the preparation of medical opinion reports, which are intended solely for use by adults in connection with VA disability claims and related matters. PVMC does not provide services to minors or on behalf of minors without the involvement of a parent or legal guardian who is the account holder.

  1. Account Registration

To use certain areas of the Site, Site User may need to create an account (“Account”). Site User agrees to provide PVMC with accurate, complete, and updated information for Site User’s Account. Site User can access, edit, and update Site User’s Account by logging into Site User’s Account on the Site. Site User is responsible for maintaining the confidentiality of Site User’s Account and password and for restricting access to Site User’s Account, and Site User agrees to accept responsibility for all activities that occur under Site User’s Account or password. Site User will have no ownership in Site User’s Account or Site User’s username. PVMC may refuse registration, cancel an Account, or deny access to the Site for any reason.

  1. Scope of Services

PVMC provides independent medical record reviews and medical opinion reports, including nexus opinion letters, for use in connection with VA disability claims (“Services”). Site User acknowledges and agrees that:

  1. all Services are based solely on a review of medical and service records provided by the Site User;

  2. PVMC does not conduct physical examinations or telehealth evaluations of any kind;

  3. PVMC does not provide legal advice, claims strategy, legal representation, or any attorney services;

  4. PVMC’s role is limited strictly to medical analysis and documentation;

  5. reports and opinions are prepared by qualified medical professionals exercising their independent professional judgment; and

  6. Services are provided on a per-condition, per-report basis as described on the Site.

  1. No Doctor-Patient or Attorney-Client Relationship

Use of the Site and engagement of PVMC’s Services does not create:

  1. a doctor-patient relationship between Site User and PVMC or any of its medical professionals;

  2. an attorney-client relationship of any kind; or

  3. any fiduciary duty, duty of care, or ongoing obligation beyond the specific Services purchased.

PVMC’s Services constitute independent medical evaluations based on record review only. Site User should not rely on PVMC’s reports as a substitute for personal medical advice, diagnosis, or treatment from Site User’s own healthcare provider.

  1. No Guarantee of Outcomes

PVMC does not guarantee any specific outcome, including but not limited to:

  1. VA disability ratings or rating increases;

  2. service connection determinations;

  3. appeal outcomes or favorable decisions;

  4. any particular finding, conclusion, or opinion in a report; or

  5. the weight or consideration given to a PVMC report by the VA, a VA examiner, or any other third party.

Payment to PVMC is for the professional time and expertise involved in reviewing records and preparing medical opinions. Payment is not contingent upon, and shall not be construed as payment for, any particular result or outcome. PVMC does not accept payment arrangements that are contingent upon claim outcomes.

  1. Fees and Payment

    1. A non-refundable record review fee is required prior to evaluation of Site User’s case.

    2. If Site User’s case is accepted for a medical opinion, fees are charged on a per-condition, per-report basis as described on the Site at the time of purchase.

    3. Payment must be made in full prior to the release of any report or medical opinion.

    4. All payments are processed through Authorize.net or such other secure third-party payment processor as PVMC may designate from time to time. Site User may only use credit cards, debit cards, or other payment methods that belong to Site User or that Site User is expressly authorized to use.

    5. Site User is responsible for the payment of any applicable sales, use, or other taxes or duties that may apply to Site User’s purchase, as determined by applicable law.

    6. Site User will not hold PVMC liable if Site User does not comply with laws related to Site User’s transactions.

  2. Refund Policy

Due to the nature of professional medical review services:

  1. All fees are non-refundable once a review has been initiated.

  2. If PVMC determines it cannot support a favorable medical opinion based on the records provided, the record review fee still applies, as professional time has been expended.

  3. The record review fee compensates PVMC for the time and expertise required to evaluate the medical records, regardless of whether a full opinion report is subsequently prepared.

  4. PVMC reserves the right to issue refunds or credits at its sole discretion in extraordinary circumstances.

  1. Case Acceptance and Discretion

PVMC reserves the right, at its sole discretion, to:

  1. decline to accept a case or write a medical opinion if it is not medically supportable based on the records provided;

  2. limit the scope of any opinion to what is supported by the medical record;

  3. refuse service to any Site User for any reason or no reason; and

  4. discontinue work on a case at any time if PVMC determines, in its professional judgment, it cannot render an honest and supportable opinion.

Site User acknowledges PVMC’s medical professionals are bound by professional and ethical obligations that require them to render only those opinions that are medically supportable, and PVMC will not produce reports containing predetermined, falsified, or misleading conclusions under any circumstances.

  1. User Responsibilities

By using the Site and PVMC’s Services, Site User agrees to:

  1. provide accurate, complete, and truthful information in all forms, intake questionnaires, and communications;

  2. submit authentic, unaltered medical records, service records, and other documentation;

  3. not request falsified, misleading, exaggerated, or predetermined conclusions in any report or medical opinion;

  4. not attempt to influence or dictate the medical findings or conclusions of PVMC’s professionals;

  5. comply with all applicable laws, rules, and regulations; and

  6. not use the Site for any unlawful, fraudulent, or unauthorized purpose.

Any attempt to influence, coerce, or direct PVMC’s medical professionals toward a specific conclusion, or any submission of falsified or altered records, may result in immediate termination of Services without refund and permanent prohibition from using the Site.

  1. File Submissions and Uploads

PVMC’s Services require Site User to upload medical records, service records, and other documentation through the Site (collectively, “Uploaded Files”). By submitting any Uploaded Files, Site User acknowledges and agrees to the following:

  1. Acceptable Formats and Size Limits. PVMC accepts Uploaded Files in the formats specified on the Site, which may include PDF, JPEG, PNG, TIFF, and such other formats as PVMC may designate from time to time. Individual files must not exceed the maximum file size indicated on the Site at the time of upload. PVMC reserves the right to modify accepted file formats and size limits at any time without prior notice.

  2. Site User Representations Regarding Uploaded Files. Site User represents and warrants all Uploaded Files: (i) are authentic, accurate, and have not been falsified, fabricated, or materially altered; (ii) do not infringe, misappropriate, or otherwise violate the intellectual property rights, privacy rights, or any other rights of any third party; and (iii) are submitted in compliance with all applicable federal, state, and local laws and regulations, including any applicable privacy and health information laws.

  3. Security of Uploaded Files. PVMC shall maintain commercially reasonable administrative, technical, and physical security measures designed to protect Uploaded Files from unauthorized access, disclosure, alteration, or destruction. Notwithstanding the foregoing, no method of electronic transmission or storage is completely secure, and PVMC does not guarantee the absolute security of any Uploaded Files.

  4. Retention and Deletion. PVMC shall retain Uploaded Files for such period as PVMC deems reasonably necessary to complete the requested Services and any related follow-up. Following case closure, PVMC shall delete Uploaded Files within a commercially reasonable period, unless retention is required by applicable law or regulation. Site User may request early deletion of Uploaded Files by submitting a written request to PVMC at the contact information provided in these Terms; however, such deletion may limit PVMC’s ability to provide further Services related to the applicable case.

  5. No Liability for File Loss. Site User acknowledges PVMC shall not be liable for the loss, corruption, or destruction of any Uploaded Files due to technical failure, system error, cyberattack, force majeure, or any other cause beyond PVMC’s reasonable control. Site User is solely responsible for retaining original copies of all records and documentation submitted to PVMC and should not rely on PVMC as the sole repository for any files.

  6. Right to Reject or Delete Files. PVMC reserves the right, at its sole discretion and without prior notice, to reject, quarantine, or permanently delete any Uploaded Files that are corrupted, unreadable, in an unsupported format, excessively large, or that PVMC reasonably believes contain malware, viruses, or other harmful code. PVMC shall have no obligation to notify Site User of such rejection or deletion, although PVMC may, in its discretion, inform Site User and request resubmission in an acceptable format.

  1. Code of Conduct

Site User agrees Site User will comply with all applicable laws, rules, and regulations, and Site User will not:

  1. restrict or inhibit any other person from using the Site;

  2. use the Site for any unlawful purpose;

  3. express or imply that any statements Site User makes are endorsed by PVMC, without PVMC’s prior written consent;

  7. attempt to gain unauthorized access to the Site, other Accounts, computer systems, or networks connected to the Site through hacking, password mining, or any other means; or

  8. interfere with or disrupt the Site or servers or networks connected to the Site.

  1. Intellectual Property

All content, reports, templates, opinion letters, methodologies, and materials produced by PVMC are the intellectual property of PVMC and are protected by applicable copyright, trademark, and other intellectual property laws. Site User acknowledges:

  1. reports and opinion letters are provided to Site User for Site User’s personal use in connection with Site User’s VA disability claim or related proceeding;

    2. Notwithstanding subsection (a), PVMC’s customer support team may contact Site User or receive calls from Site User by telephone solely for the purpose of providing general information about PVMC’s process and services as described on the Site, taking general messages, assisting with administrative matters such as resending invoices or reconnecting returning clients with their assigned expert, and other generalized assistance of a similar nature. Customer support personnel will not discuss case-specific medical opinions, report findings, or case strategy by telephone. All telephone calls with PVMC’s customer support team are recorded for compliance and quality assurance purposes, and callers will be notified of such recording prior to the commencement of any discussion.

    3. For the avoidance of doubt, PVMC does not provide case-specific consultations, medical opinion discussions, or detailed case updates via telephone. All such communications must be conducted in writing in accordance with subsection (a) above. This policy exists for compliance, documentation, and quality assurance purposes.

    4. Site User agrees to check Site User’s Account and email regularly for communications from PVMC regarding Site User’s case.

  2. Links to Third-Party Websites

The Site may contain links to other websites that are not owned or operated by PVMC. The fact PVMC may link to those websites does not indicate any approval or endorsement of those websites. PVMC has no control over those websites. PVMC is not responsible for the content of those websites or the privacy practices of those websites. PVMC strongly encourages Site User to become familiar with the terms of use and practices of any linked website. It is up to Site User to take precautions to ensure whatever links Site User selects, or software Site User downloads (whether from the Site or other sites), is free of viruses, worms, Trojan horses, defects, and other items of a destructive nature.

  1. Privacy

PVMC respects Site User’s privacy and is committed to protecting Site User’s personal information. PVMC’s collection, use, and disclosure of Site User’s personal information is governed by PVMC’s Privacy Policy, which is incorporated into these Terms by reference. By using the Site, Site User consents to the collection, use, and disclosure of Site User’s personal information as described in the Privacy Policy. PVMC does not control how third parties evaluate, weigh, or consider medical evidence, including reports and opinions prepared by PVMC. Site User releases PVMC from any claims and damages, known and unknown, arising out of or in any way connected with any claim Site User has against any such third parties.

  1. Violation of These Terms

PVMC may investigate any violation of these Terms, including unauthorized use of the Site. PVMC may take legal action it considers appropriate. Site User agrees that monetary damages may not provide PVMC a sufficient remedy and that PVMC may pursue injunctive or other equitable relief for Site User’s violation of these Terms. If PVMC determines that Site User has violated these Terms or the law, or for any other reason or for no reason, PVMC may cancel Site User’s Account, delete all Site User’s information, and prevent Site User from accessing the Site at any time without notice to Site User. NOTWITHSTANDING THE FOREGOING, NOTHING IN THESE TERMS SHALL EXCLUDE OR LIMIT PVMC’S LIABILITY FOR LOSSES OR DAMAGES WHICH MAY NOT BE LAWFULLY EXCLUDED OR LIMITED BY APPLICABLE LAW, INCLUDING MANDATORY CONSUMER PROTECTION RIGHTS UNDER TEXAS LAW.

IN NO EVENT SHALL PVMC’S TOTAL AGGREGATE LIABILITY TO SITE USER FOR ALL CLAIMS ARISING OUT OF OR RELATED TO THESE TERMS OR THE SERVICES EXCEED THE TOTAL AMOUNT PAID BY SITE USER TO PVMC IN THE TWELVE (12) MONTHS IMMEDIATELY PRECEDING THE EVENT GIVING RISE TO THE CLAIM.

SITE USER’S SOLE REMEDY FOR DISSATISFACTION WITH THE SITE OR THE SERVICES IS TO STOP USING THE SITE AND THE SERVICES.

  1. Indemnification

PVMC reserves the right, at PVMC’s own expense, to assume exclusive defense and control of any matter otherwise subject to indemnification by Site User and, in such case, Site User agrees to cooperate with PVMC in the defense of such matter. Site User may not settle any claim covered by this Section or admit any liability on the part of PVMC without PVMC’s prior written approval.

  1. Dispute Resolution

Any dispute, claim, or controversy relating in any way to these Terms, to Site User’s use of the Site, or to any Services provided by PVMC will be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of laws principles. Site User and PVMC agree exclusive venue and jurisdiction for any legal action or proceeding arising out of or relating to these Terms shall be in the state or federal courts located in Bexar County, Texas, and each party irrevocably consents to the personal jurisdiction of such courts.

It is agreed any dispute resolution proceedings will be conducted on an individual basis only, and not in a class, consolidated, or representative action. SITE USER ACKNOWLEDGES AND AGREES  SITE USER AND PVMC ARE EACH WAIVING THE RIGHT TO A TRIAL BY JURY OR TO PARTICIPATE AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS ACTION, COLLECTIVE ACTION, OR REPRESENTATIVE PROCEEDING.

PVMC’s failure to exercise or enforce any right or provision in these Terms will not constitute a waiver of that right or provision or any other rights or provisions included within the Terms.

  1. Modifications to These Terms

PVMC reserves the right to update, amend, or modify these Terms at any time in its sole discretion. Any changes will be effective immediately upon posting the revised Terms on the Site, with the “Last revised” date updated accordingly. Site User’s continued use of the Site after the posting of revised Terms constitutes Site User’s acceptance of such changes. PVMC encourages Site User to review these Terms periodically for any updates.

active duty, was commissioned as an officer, and subsequently deployed to Afghanistan.

Alan’s military and medical background inspired him to form Prestige Veteran Medical Consulting with his
wife, Leah Bucholz, a Physician Assistant and Army Combat Veteran.  He has devoted himself to using his
knowledge gained in the military as a medical professional to serve the Veteran community.

Leah - Meet the Team

LEAH BUCHOLZ, PA-C

U.S ARMY VETERAN, MEDICAL EXPERT & FORMER C & P EXAMINER

Meet Leah, the founder, and leader of our organization. A combat veteran herself, she understands the unique challenges veterans face, making her mission about much more than running a successful business. It’s about the opportunity to provide a legacy of exceptional service for our heroes.

“It is essential to remember that there is no greater honor than caring for service members on the battlefield. Continuing to care for Veterans after separation is an opportunity that I have been afforded to extend that care in this new battlefield related to service-incurred disabilities.”

Her inspiration comes from years of military experience working alongside her fellow servicemembers, particularly key leaders who have influenced her journey, applying their wisdom to her path. She values her team deeply, most of whom are also disabled combat veterans, each member a trusted individual sharing her vision.

 “Driven by passion and purpose, I aim to create a sustainable change that empowers veterans and future generations.”

Leah’s journey hasn’t been without challenges. From overcoming and living with her service-related disabilities to navigating work-life balance, she’s learned to face each hurdle head-on with resilience.  Like many of her veteran brothers and sisters, her early background consisted of limited resources and opportunities. Her military service has helped shape her into a steadfast leader, offering relatable inspiration to others.

Join us in celebrating Leah, a compassionate leader, and resilient veteran, driving our mission to serve those who served our nation.
